A Collaborative View of Connected Health: Payers, Providers, Technologists

“Connected Health” is a healthcare delivery model where technology is used to connect various silos in healthcare delivery system. Simply put, it aims to engage providers and empower patients with the right care at the right time and right place, with the consideration to reduce cost and improve quality.

Connected health can mean different things to different people.  We have seen disparate trends in this fragmented market.  On one end of the spectrum, the consumerization of healthcare, which essentially is wellness or fitness management, in the form of wearable gadgets such as FitBits; on the other end, we face a huge population of chronically ill patients that could potentially benefit from the Connected Health model.

For tonight’s panel, we will focus on how to use Connected Health to support chronic disease management. Three main areas we will discuss tonight:

  • As we transition from the existing fee-for-service to the population management based/ACO reimbursement model, and from the reactive to proactive and preventative care delivery model, what changes are being called for, i.e. mindset, delivery chain, underlying support infrastructure? What and how technology opportunities can be leveraged to enable and accelerate this progression? (or better termed as) Challenges?
  • When, what and how information is required to send to provider and / or patient – goal is to get them to work together (an Integrated delivery system – ACO)?  Specifically, what is the role of meaningful analytics in making a difference for Connected Health devices and services?
  • What are the emerging pathways to payment / reimbursement around Connected Health devices / services?

Principles: Ask questions to stimulate discussion and solicit practical solutions from panelists that can address current state of issues in healthcare.  Stay away from far-off scenarios.

PANELISTS:
Dr. Tariq Dastagir, MD
Physician Informaticist/Medical Director, Anvita Health/Humana Inc.
With a solid background in clinical care and innovations in clinical informatics, Tariq Dastagir has successfully initiated and helped implement innovations in electronic health records and population health management. Tariq holds board certification in internal medicine, has completed a fellowship in clinical informatics with distinction and has experience as both a clinician and clinical informaticist, He is currently working as medical director  for clinical data analytics for a Fortune 100 company. After he completed his internal medicine residency, he worked as hospitalist and primary care provider. It was while working as a front line physician and leading CPOE and EMR implementation that he developed a deep interest in the implementation and study of clinicians’ attitudes towards clinical computing tools. His research interests included change management around EHR implementation and clinician training for optimal EHR utilization for providing improved quality care while enhancing physicians’ productivity. He is also working on population segmentation for targeted and focused interventions, improving clinical outcomes while reducing costs. Tariq’s passion is to revolutionize healthcare industry by helping to develop the most actionable data analytic product with effective use of big data, artificial intelligence, machine learning and usability principles, to help clinicians deliver higher quality, cost-effective care at both population and individual level.

Dr. James Killeen, MD
Associate Professor of Emergency Department & Hyperbaric Medicine Department, UCSD,
Director of Information Technology Services, UCSD Emergency Department

James Killeen, MD is a UCSD Clinical Professor for both the Emergency Department and Hyperbaric Medicine Department. He is Director of Information Technology Services for the UCSD Emergency Department. Chief Medical Officer for the San Diego Health Connect. His accomplishments include the design & implementation of the first UCSD Health Information Exchange (HIE) that links community clinics with UCSD for clinical records and medical homes; the design & implementation of the San Diego EMS Hub that links EMS patient records & EKGs to hospitals in real-time; and the lead architect for the ONC funded San Diego Beacon Project which has established first San Diego Regional Health Information Exchange.Dr. Brenda Fischer, PhD, RN, MBA, CPHQ

Dr. Fischer is known as a visionary, strategic thought leader and self proclaimed Possibilitarian. She brings a balanced entrepreneurial, academic and service perspective with an energized outcomes focus. She has successfully developed collaborative thinking partnerships with the goal of advancing comprehensive transformation in health care. A key partnership model involved designing translational technology research and development systems that were implemented in the “living laboratory” clinical environment. This model facilitated FDA approval for enabling technology and the design of care delivery systems to improve patient outcomes. Dr. Fischer currently serves on the Boards of the City of Hope, Southern California Cancer Pain Initiative; University of San Diego, Hahn School of Nursing Alumni; and the California State University Institute of Palliative Care, Advanced Practice Nursing Graduate Program at San Marcos. Dr. Fischer was recently appointed to the American Organization of Nurse Executive’s committee that developed the 2013-2016 Strategic Plan for this professional organization. She founded the Center for Nursing Excellence, a think tank and action center at Palomar Health, and served as the Director and Magnet Program Director with a key result of successfully leading this health system to Magnet Recognition by the American Nurses Credentialing Center. Dr. Fischer is the recipient of the Award for Excellence in Pain Management and Palliative Care Leadership by the Southern California Cancer Pain Initiative and the Patriot Award by the Department of the Defense Support of the Guard and the Reserve.Dr. Fischer is a national presenter and a member of numerous professional organizations. Dr. Fischer’s service specialties and academic research focus includes Leadership, Positive Organizational Scholarship, Complex Health Populations, Systems Transformation, Healthcare Delivery Models, Optimal Healing Environments, System Magnet Recognition, Interprofessional Research/Program/Grant/Philanthropy Models, Academic/Service Partnerships, Performance Excellence, Patient Safety, Electronic Health Record Design and Implementation, Predictive Analytics, Business Intelligence, as well as the use of large administrative and clinical data bases for research.Dr Fischer has a PhD in Nursing from University of San Diego with a focus on Executive Leadership and Education. Her Dissertation was on Predictors of Rehospitalization in Home Healthcare Patents. She has a Master of Science in Business Administration, a Master of Science in Nursing Administration and a Bachelor of Science in Nursing from California State University institutions in Southern California.Ed Leer
President, WANDA, Inc.

Mr. Leer is president of WANDA, Inc., a predictive modeling software company focused on remote monitoring sensor data. Developed at UCLA’s Wireless Health Institute and tested throughout the U of CA’s five medical centers, this analytic engine has proven to reduce readmissions and efficiently manage nurse triage/monitoring resources. Prior to WANDA, Mr. Leer was president of Profficee, Inc., a Healthcare Service Company, whose services included consulting, technology, revenue cycle and practice management. Mr. Leer has lead consulting services, including interim CIO, to many leading hospitals and large physician groups across the US. Mr. Leer also served as CIO of a venture capital funded company focused on home health care remote monitoring using cellular technology. Previously, Mr. Leer was one of the four founding members of ONEX, Inc, an information technology consulting company that grew to over 200 consultants within its first three years before being acquired in 2001 by Inrange Technologies. Prior to ONEX, he served as CIO for Acordia Inc., and VP of IT for its parent company, Anthem (now Wellpoint), where his responsibilities for system decisions were critical to their corporation’s $10 billion business. Ed also has 12 years’ experience with AT&T including the roles of Regional Product Marketing Manager and Midwest Regional Technical Manager. Ed’s education includes a B.S. in Accounting from Indiana University and a Computer Systems fellowship from MIT in a joint AT&T/MIT program. Mr. Leer was chair of the Indiana HIPAA Consortium (IHC), a group of leading Payers and Providers, collaborating their HIPAA efforts in order gain economy of scales and quality improvements over their individual efforts. Mr. Leer was a panelist at the Fourth National HIPAA Summit in Washington DC and the Fifth National HIPAA Summit in Baltimore discussing the State’s compliance activities. Ed served on the Board of Advisers for IU’s School of Nursing, along with past Board appointment on Purdue’s school of Engineering and Technology at Indianapolis. He also co-chaired the technology committee for the Central Indiana Coalition to Reinvent Healthcare, which evolved into Indiana’s leading HIE, Indiana Health Information Exchange (IHIE).


Dr. Jeff Benabio, MD
Director of Healthcare Transformation, Kaiser Permanente, MBA Candidate, Rady School of Management, UCSD
Dr. Benabio is Physician Director of Innovation and Transformation at Kaiser Permanente, San Diego, CA, where he leads several projects, including social networking for physicians, lean initiatives, and technology-enhanced healthcare delivery. He is a board certified dermatologist and member of the American Academy of Dermatology.

In San Diego, he helped create and sustain one of the largest telemedicine programs in the country. He has been an example and advocate for using social networks to educate and empower patients and has become a sought after speaker, lecturing on telehealth, health innovation, and social networking for health systems and physician groups. A consultant for telemedicine and digital start-ups, he serves on several boards, including The American Telemedicine Association and the External Advisory Board for the Mayo Clinic Center for Social Media.

Dr. Benabio is the voice behind The Digital Doctor (http://thedermblog.com/), a patient-centered blog about skin health and general wellness as well as a new website, Jeff Benabio MD, Healthcare Happiness (http://benabio.com/), which focuses on broader issues such as the changing face of healthcare, healthcare and technology, and the doctor/patient relationship.

Dr. Benabio writes a monthly column on technology and healthcare for Skin & Allergy News, and is frequently quoted both in print and online in publications such as O, The Oprah Magazine, Health, The New York Times, and WebMD. Perhaps, most important, Glamour magazine named him “Rock Star of Dermatology,” making him his niece’s coolest uncle.
